WHAT IS IT?
The temporomandibular joint (TMJ) connects the mandible, which is the jaw bone, to the temporal bone which is part of the skull. Between these two bones, inside the joint, there is an articular disc formed of fibrous cartilage which allows the mandible to slide forward to fully open the mouth. The muscles in the temples and on the sides of the jaw close the mouth; we feel them working when we grit our teeth together.
WHAT'S THE CAUSE?
Articular disc dislocation
It is possible that the articular disc between the mandible and the temporal bone moves forward and gets stuck there. In such a case, we speak of a disc dislocation. If during the opening of the mouth, the disc returns to its place, this displacement produces an audible "click" and the dislocation is then called "reducible". On the other hand, in the case of a so-called “non-reducible” dislocation, the disc remains stuck in this position and limits the opening of the jaw and therefore the mouth. Depending on how the disc is compromised, it is possible that the mouth may remain stuck open with difficulty closing it.

Referred cervical pain
Jaw pain can also arise from the structures that make up the cervical spine (the neck). In such an instance, we speak of “referred” pain, since the pain felt is in a part of the body that has no lesion or injury. These pains can occur without neck pain, and are sometimes accompanied by headaches, tinnitus, shoulder blade and shoulder pain, as well as numbness. Often, if the pain is coming from the neck, there is no painful click or limitation of the opening of the mouth ; only pain. Many persistent jaw problems have a cervical component that has never been identified, so it is important to determine if the neck is a factor in your problem. |
At Active Solution Physiotherapy in Montreal, a scientific evaluation procedure based on a cause and effect relationship will be able to clearly demonstrate whether your pain has a cervical component or not.
Bruxism
We call “bruxism” an involuntary grinding or clenching of the teeth. It can be caused by a lack of sleep, anxiety and stress, or a problem with the alignment between the upper and lower teeth. It can cause local pain, headaches, as well as tooth sensitivity to pressure or temperature (hot or cold).

HOW CAN PHYSIO HELP?
For effective treatment of TMJ problems, it is important to determine the structures causing the symptoms. The neck and TMJ should be examined to identify the exact source of the symptoms. Active Solution Physiotherapy uses the McKenzie Method/Mechanical Diagnosis and Therapy (MDT), a systematic approach that helps to effectively classify and understand the cause of your jaw problem.
